How to use your Healthcare Plan

  1. How to make a house call appointment: please contact our MEDICARE Customer Support Line – (+351) 219 441 113, option 1, followed by option 3, and carefully follow instructions. You will need your MEDICARE MAIS GIFT card.
  2. How to request Health Advice Over The Phone: please contact our MEDICARE Customer Support Line – (+351) 219 441 113, option 1, followed by option 2, and carefully follow instructions.
  3. Choosing a Dentistry Service Provider: visit www.medicare.pt click on Medical Network, and select the Healthcare Plan from which you accessed this Campaign. Search for the intended Service provider.
  4. Making a dentist appointment in the MEDICARE Network: please contact the MEDICARE dental appointment line – (+351) 211 165 530 – and we will make an appointment with your preferred Service Provider. This dental appointment line is available every workday from 9 a.m. to 8 p.m.
  5. What to do during an appointment: you should always show your MEDICARE MAIS GIFT card to the chosen Service Provider together with a personal identification document containing a picture. After each appointment, you will be required to pay for the acts from which you have benefited, that is, you will be required to pay the agreed sum between MEDICARE and the Service Provider. You should always ask for a receipt to be included in your IRS tax return.
